URGENT - PLEASE SEE LATEST NEWS REGARDING CHANGE TO COMPUTER SYSTEM
Over the next few months we will be introducing some improvements to the way you access services at Caradoc Surgery. A national programme called Care Navigation, is being introduced across the country, and there will be some changes to ensure you are able to see the most appropriate service for your specific needs, as quickly as possible. Our new telephone system and single point of contact will also be launched in conjunction with this, providing 30 new lines and a state-of-the-art call queuing system to improve access to patients, especially at peak times.
Care Navigation is a process that has been designed by doctors and other clinical staff to ensure that patients are supported to access the right service for their needs. It involves trained staff called ‘Care Navigators’ to ask patients requesting an appointment a few questions to ensure that they get the right service for them. For example, if a patient needs to see a specialist nurse or other healthcare service, and it is not necessary for them to see the GP first, they can be referred directly and the Care Navigator will make sure this happens – saving yours and the doctors’ time.
Some patients are not aware that they don’t always need an appointment with their doctor. In fact, many of the problems that patients have can be seen by a nurse prescriber or other clinical staff at the surgery. There may also be options for patients to take care of themselves, through the advice of their local pharmacist or other healthcare service provider. This can often be the quickest and best route for the patient and save the doctors’ time to deal with things that only they can help with.
We will keep you updated with information on progress over the coming months as we introduce these changes to the services provided. Please ensure you visit the surgery website or come in to the surgery for updates on how these improvements are progressing and what this means for how you access services at Caradoc Surgery.
Did you know that you can now manage many services without having to phone or come in to the practice?
Some things can be easily done online, such as:
- Booking appointments
- Repeat prescriptions
- See your summary care record (allergies, vaccinations etc.)
To register for this service just bring one form of photographic ID (passport / driving license) into the practice and ask the receptionist to register you and provide you with your username and password (which you can then change when you log in for the first time). You can also request access to your medical records online, please ask at reception for a separate application form.
